how do they get them without the cage infront of the lens?

that ones not taken by me - its just in my favourites on that site lol

but the one time i have taken photos when a cage has been used i lent the end of the lens on the cage wire and followed the fighters around from that position - or you can zoom in if your using 1 point/centre focus and hope for the best - but the mesh can play havoc when focusing and dark venues dont help!

depending on the lighting above/around the cage there can be a shadow cast!
